The Town of Stratford Health Department is seeking a Public Health Nurse (Part-Time)
The Public Health Nurse provides professional nursing services that support the health and well-being of individuals, families, and the community. This position plays a key role in delivering preventive health services, improving access to care, supporting public health programs, and responding to community health needs. The Public Health Nurse works collaboratively with residents, healthcare providers, community organizations, and public health staff to promote wellness, prevent disease, and reduce health disparities.
Key Responsibilities Include:
- Provide nursing services through community-based programs, clinics, and outreach activities.
- Administer vaccines and provide immunization services in accordance with state and federal guidelines.
- Conduct health screenings, assessments, and educational sessions to promote healthy behaviors and early identification of health concerns.
- Provide public health nursing support for communicable disease prevention, investigation, follow-up, and reporting.
- Perform home visits and community outreach to assess health needs, provide education, and connect residents with appropriate resources.
- Educate individuals and families on disease prevention, chronic disease management, medication safety, wellness, and available community resources.
- Support public health initiatives related to emergency preparedness, response activities, and community health planning.
- Participate in health fairs, outreach events, vaccination clinics, and other community engagement activities.
- Maintain accurate clinical documentation, data collection, and required reporting in accordance with public health standards.
- Collaborate with internal staff, healthcare providers, social service agencies, schools, and community partners to coordinate care and improve health outcomes.
- Provide compassionate, culturally responsive care while addressing barriers that impact access to healthcare services.
Qualifications:
- Current Connecticut Registered Nurse (RN) license required.
- Bachelor’s degree in Nursing preferred; public health nursing experience strongly preferred.
- Experience providing community-based nursing services, health education, immunizations, or preventive care preferred.
- Strong communication, organization, assessment, and critical-thinking skills.
- Ability to work independently while also collaborating effectively with a multidisciplinary team.
- Ability to travel within the community for outreach, home visits, and program activities.
- Commitment to improving population health and supporting equitable access to healthcare services.

The Chesprocott Health District is seeking a full-time Public Health Nurse (PHRN) to oversee and manage public health clinic operations serving the district community. The role focuses on adult and pediatric immunizations, communicable disease investigations, vaccine management, lead case reporting, and public health education.
