Public Health Nurse Consultant (Full time 35hr/wk)

The State of Connecticut, Department of Public Health (DPH) is seeking a qualified individual to join our team as a Nurse Consultant. This position will support our Health Care and Support Services (HCSS) Unit’s Ryan White Part B Program and will report to the Public Health Services Manager. Serving as the Quality Management Coordinator, this role will lead our Quality Management activities required under the Health Resources and Services Administration (HRSA) Ryan White Part B grant.

MINIMUM QUALIFICATIONS:

  • Four (4) years of experience as a professional nurse in a position involving supervision, consultation, education or administration.
  • Two (2) years of the General Experience must have been in a public health nursing program, home health care agency, community health care setting, or in a hospital.
  • A Bachelor’s degree in nursing may be substituted for one (1) year of the General Experience.
  • A Master’s degree in health services administration, nursing or public health may be substituted for one (1) additional year of the General Experience.

PREFERRED QUALIFICATIONS:

  • Experience working in both clinical and public health organizations.
  • Experience with implementing Quality Assessment, Quality Improvement and Program Evaluation in a public health, infectious disease, or related health care setting.
  • Experience developing, facilitating, and presenting on the principles of Continuous Quality Improvement (CQI), e.g., brainstorming, cause and effect analysis, root cause analysis, and graphic representation of data.
  • Experience developing collaborative projects and working with a variety of public and/or private providers, policymakers, researchers, and persons with lived experience, such as HIV, Viral Hepatitis, and STDs.
  • Experience demonstrating advanced communication, analytical, and organizational skills.

Community Health Nurse (Full time 37.5hr/wk)

$80,000 – $85,000/Annually

SUMMARY OF POSITION: Under general direction of the Director of Health, plans, supervises, and provides public health services of a clinical, research and/or educational nature.

DESIRABLE KNOWLEDGE AND SKILLS: Provides required investigation and follow-up of communicable disease reports, makes referrals to Manchester Public Health Nursing Association for home visits, conducts studies of suspected disease outbreaks, serves on infection control committees of hospital and nursing homes. Assists with needs assessments, epidemiological or statistical studies relating to the health of the community, consults with the medical advisor and other Human Services Department staff regarding study topics, methods, and results. Thorough knowledge of current community nursing principles and practices. Ability to analyze data to determine patterns of health and illness in the community and recognize the impact of various environmental, cultural and social influences of these patterns. Assists contract physician to provide employee health services, set up clinics, gathers information for pre-employment physicals and Workers’ Compensation cases. Provides nursing services such as giving tetanus boosters to employees.

MINIMUM QUALIFICATIONS: Bachelor’s degree in Nursing or related field with an active RN license to practice in Connecticut and a minimum of two (2) years’ of experience in a public or community health program preferred.

Occupational medicine experience a plus. Suitable experience may be substituted for educational attainment if deemed appropriate by the General Manager or his designee. Bilingual preferred, but not required.

Part Time Public Health Nurse

Town of Glastonbury

GENERAL DESCRIPTION:

Under broad supervision, incumbent plans, implements and promotes the Town’s health promotion and disease prevention programs.

ESSENTIAL DUTIES:

1. Evaluates the health care needs of residents through assessments and screening. Provides treatments consistent with the orders of the Town’s Medical Director or Commissioner of Connecticut Department of Public Health. Assists with needs assessments, epidemiological or statistical studies relating to the health of the community.

2. Plans, coordinates, conducts and evaluates group health promotion activities such as immunization clinics, screenings and educational programs.

3. Promotes community clinic and health services through press releases, flyers, posters, social media, radio announcements, group presentations and colleagues throughout the community.

4. Conducts infectious disease control activities such as contact tracing, infectious case interviews, and health monitoring of identified contacts.

5. Participates in the investigation of outbreaks of illness and disease which may be caused by environmental factors in consultation with other Health Department staff.

6. Coordinates information sharing, reporting, and mutual assistance with state partners at Connecticut Department of Public Health and other local health departments.

7. Develops, presents and publicizes health education and technical information for groups and individuals with diverse public health needs, including guides, manuals and other teaching aids.

8. Maintains confidential, accurate case health records and statistical data for reporting, analysis and trend identification. Prepares reports and significant findings for the Director of Health, Connecticut Department of Public Health, and others as appropriate.

9. Participates in the management of grant deliverables by tracking activities, preparing deliverables, and reporting for grant-supported programs.

10. Responds to local and regional public health emergencies and participates in emergency preparedness planning, training, drills, and exercises.

11. Contributes to a respectful and safe work environment by fostering a culture of positive communication and ethical behavior, with an emphasis on providing exceptional service to internal and external customers.

MINIMUM TRAINING AND EXPERIENCE:

  • Bachelors’ degree from an accredited college or university in nursing, public health or health-related field. Master’s degree in public health, preferred
  • Two (2) years’ experience in a public or community health program
  • Public speaking experience
  • Experience and/or certification in public health emergency preparedness planning and response desired

The primary responsibility of the Public Health Nurse is to perform the necessary duties in fulfillment of deliverables set

Public Health RN opportunity at Eastern Highlands Health District

Part time position, 20 hours per week. 😷

The Eastern Highlands Health District (EHHD) is seeking a detail-oriented, motivated, and dynamic individual to serve as a part-time public health nurse. Primary responsibilities include supporting the local health department efforts prevent illness and promote wellness. This may include but is not limited to vaccination clinic coordination, vaccine administration, health education and education event coordination, case contact investigations, providing health advice and guidance to individuals, families, and community stakeholders, and other public health activities as needed. This position reports to the Community Health and Wellness Coordinator. This is a durational, grant funded appointment.

The hourly rate is $37 to $41 per hour.
